That voice scared the shit out of me too. It’s just a homeless NPC bug:

H3 - Easter Eggs (EEs)

This is a problem with how the audio is handled for a homeless NPC vocal line. It is set to max loudness (distance doesn’t make the voice audio quieter). There is a similar problem with the Undercover agent in berlin near the crane. When he speaks to the two sitting guards they have a conversation and the line “That’s Bullshit” can be heard at maximum loudness but everything else is affected by your distance. This is a bug and not an Easter Egg.
